Do you have to fill out two separate w-4's for different locations of the same store?

I work for a small yogurt company that just opened a second location under the same owners in a different city. I've been working there a couple days a week until they hire a manager for full time. In order to get paid from both stores would I have to fill out a second w-4 form if they already have had me in their system for a year and none of my information has changed?
